Appointments & Cancellations

Office hours are by appointment, Monday through Thursday from 8am – 5pm with lunch from 12 – 1pm. Your scheduled appointment time has been reserved exclusively for you. We require at least 48 hour notice, if you need to cancel your appointment. Missing appointments without adequate notice, frequent rescheduling or cancellations will result in no longer being eligible for services.

Payments & Insurance

Payment for professional services is due at the time of treatment. For your convenience, we accept cash and most major credit cards: MasterCard, Visa, and Discover Card. We offer financing through Care Credit and Lending Club for more extensive restorative treatment cases.

Our office is committed to help our insurance patients with covered services. Since insurance policies vary, we can only estimate your benefits and cannot guarantee coverage. Benefits are determined by the insurance policy issued by your employer or individual plan. As a courtesy, we will file your insurance claim for the estimated covered services. Your estimated amount must be paid at the time of service.
